115年度『高英士研究獎學金』發放辦法（如附件），敬請符合資格之碩博士生於9/23(三) 17:00前將申請資料繳至系辦，謝謝您!

申請流程：

1. 請先填寫線上表單：https://forms.gle/XKLuH5YpG6xqtQmx6
2. 並備妥以下紙本文件，於9/23(三)下午5點前繳至系辦周小姐：

A. 正式歷年成績單（請至教務處申請）

B. 未得其他奬學金證明（請至學務處申請）

C. 戶口名簿影本或全戶戶籍謄本 (外籍生請檢附居留證與學生證)

D. 專題報告

E.『高英士獎學金』申請表，如附件

F. 化學系獎學金申請檢核表，如附件

★請注意：以上兩項步驟（線上表單填寫及紙本資料繳交）皆須於期限內完成，如未完成任一步驟，視為未完成申請。  

Dear Students,

Please see the attached “高英士 research scholarship” award guidelines for 2026. Eligible Ph.D. students who wish to apply for the scholarship please submit their application documents to the department office by 5:00 PM, Wednesday, September 23.

Application process:

1. Please first complete the online form: https://forms.gle/XKLuH5YpG6xqtQmx6
2. Then prepare the following paper documents and submit them to the department office Ms. Wendy Chou by 5:00 PM, Wednesday, September 23:

A. Official Chinesetranscript of academic records (apply at the Office of Academic Affairs)

B. Certificate of not receiving other scholarships (apply at the Office of Student Affairs)

C. A copy of Student ID card and ARC card (for international students)

D. Research report

E. “高英士 research scholarship” application form (see attachment)

F. Department of Chemistry Scholarship Application Checklist (see attachment)

★Please be aware:  Both steps above (completing the online form and submitting the paper documents) must be completed by the deadline. Failure to complete either step will be considered an incomplete application.
